Provides intermittent skilled nursing, physical therapy, occupational therapy, speech therapy, medical social service, and home health aide to homebound Baltimore County residents under the direction of their physician. Also, provides for prompt evaluation and follow-up of Baltimore County residents who are hospitalized and works with the patient, family and attending physician to plan the patient's post-hospital needs. Refers patients to other services such as Home Health, clinics, home care services, nursing homes and case management.

In-home aides help qualified elderly, chronically ill, or disabled persons remain in their homes by helping them with some simple daily activities such as bathing, dressing, feeding, grooming, and toileting. This program is designed for medically eligible Medical Assistance / Medicaid recipients.

Public health nurses provide clinical and case management services to at-risk populations including high-risk infants and toddlers in areas such as: well child clinics, family planning, immunizations for infants and school-age children, HIV/AIDS, screening for cancer and hypertension.
